LIABILITY RELEASE, & CLASS PARTICIPATION AGREEMENT

Please read carefully and tick the consent box to say you agree.

This form applies to all yoga classes offered by Charlotte May (referred to as “the Instructor” or “Lottie May Yoga”) at any of my class venues.


1. Liability Release

By joining my classes, you confirm that you are physically fit for your yoga class and that you will notify your instructor if your medical condition changes. You also confirm that you have informed your instructor of any medical conditions, injuries or medication you are taking that could affect your practice or that could affect other participants in the class.

You will also notify your instructor of any changes to your health that could affect your practice.

Please do notify your teacher if you are pregnant or have recently delivered a baby. Not all classes are suitable for the prenatal and postnatal period, so please enquire with your instructor first to make sure your class is appropriate.

Lottie May Yoga does not accept liability for any injury t o self or for any loss of personal property.


2. Class Participation Agreement

To ensure a safe, respectful, and enjoyable environment for all, I agree to:

Book my place in class using the online booking systems.

Notify the Instructor in advance if I am running late for a class or am unable to attend.

Abide by a 24 hr cancellation policy, understanding that any late cancellations are not eligible for a refund or able to be rescheduled.

Turn off or silence mobile phones during class to avoid disruption. Please refrain from taking any photos or videos taken during the yoga class.

Inform the Instructor before class if I am experiencing any pain, injury, fatigue, am pregnant or or have any unusual symptoms.

Follow the Instructor’s guidance and listen to my own body, modifying or stopping when needed.

Treat all participants and the Instructor with kindness, patience, and mutual respect.
